THE HUMAN SERVICES DEPARTMENT AND THE ELDERLY AND VETERANS SERVICES DIVISION RECOMMEND APPROVAL OF THE ABOVE MOTIONS.
These actions support the Commission Value of "Approaching human services collaboratively and compassionately, with special emphasis on the most vulnerable;" and the Goals to "Effectively advocate for and acquire municipal, state and federal financial support to equitably address health and human services needs of the entire community, through a truly coordinated system of care;" and to “Collaborate with public and private partners to find creative, equitable, and responsible solutions to systemic community problems, especially permanent, supportive housing for persons experiencing, or at risk of, homelessness.”
The Elderly and Veterans Services Division (EVSD) provides case management, health support, in-home and adult day care services to frail elders. Eligible functionally and/or cognitively impaired community-dwelling individuals, 60 years of age and over, receive appropriate elder care assistance to prevent, postpone or reduce their placement in nursing homes and other institutions.
Effective date of this Agreement is retroactive due to delays with obtaining necessary documentation from the funder. Each County Fiscal Year, the Areawide Council of Broward County, Inc. (Council) makes local match funding available for case management and adult day care services to eligible seniors without requiring their qualification for the state-funded Community Care for the Elderly (CCE) Program. In-home and other community services are made available to eligible recipients with the Council’s prior authorization. This Agreement provides one flexible spending allocation category for all funded contract services. The single spending authority simplifies EVSD's ability to earn revenue from reimbursement of all service units. This agreement provides a total of $593,106, which consists of the $360,676 2016-2017 allocation and $232,430 unrestricted carry-over from the 2015-2016 agreement. The Council is solely responsible for determining client eligibility and releasing eligible clients to EVSD for the provision of services.
The Council is not requiring EVSD to serve a specific number of seniors or to meet any other performance measures. The Council is, however, requiring that EVSD ensure that persons served are not recipients of the same services through the CCE Program or the Statewide Medicaid Managed Care Long-Term Care Program. While no performance measures are set by the Council, EVSD will utilize and apply the same performance measures outlined for the CCE program as follows: 66% of clients demonstrate improvement in Nutrition score; 63% of clients demonstrate improvement in Activities of Daily Living score; 62% of clients demonstrate improvement in Instrumental Activities of Daily Living score; 90% of Caregivers report ability to continue providing care; 74% of clients demonstrate improvement in Environment score; the average time in this program for probable Statewide Medicaid Managed Care Long-Term Care clients shall be less than 2.8 months. These goals were met in all previous grant years.
This Agreement is in the Council's format and as such their representative will sign last following approval by the Board of County Commissioners.
