A. MOTION TO RETROACTIVELY APPROVE Vendor Agreement CB00006-15-2014 between Areawide Council on Aging of Broward County, Inc. and Broward County, effective retroactive from October 1, 2013 and continuing through September 30, 2014, providing an allocation of $360,676 for the purpose of establishing a local match contract for the provision of case management, in-home and community-based services for frail elders and creating a single spending authority for all services; and authorizing the Mayor to execute the same.

B. MOTION TO ADOPT unanticipated revenue Resolution No. 2014-287 within the Local Match Vendor Agreement for the Elderly and Veterans Services Division in the amount of $360,676 to the Local Match Program Grant Fund for case management and adult day care services.

Summary Explanation/ Background
THE HUMAN SERVICES DEPARTMENT AND THE ELDERLY AND VETERANS SERVICES DIVISION RECOMMEND APPROVAL OF THE ABOVE MOTIONS.

These actions support the Commission Vision of a "Social Safety Net: An Accessible collaborative approach to health and human services" and the Goal to "Coordinate and maintain quality accessible health and human services with public and private partners."

Effective date of Vendor Agreement CB00006-15-2014 is retroactive due to delays with obtaining necessary documentation from the funding agency. Elderly and Veterans Services Division (EVSD) did not receive the final revised version of this contract from the Areawide Council of Aging of Broward County, Inc. (Council) until April 2, 2014.

EVSD provides case management, health support, in-home and adult day care services to frail elders. Eligible functionally and/or cognitively impaired community dwelling individuals, sixty years of age and over, receive appropriate elder care assistance to prevent, postpone or reduce their placements in nursing homes and other institutions.

Through this Vendor Agreement, the Council makes local funding available for case management and adult day care services to eligible seniors without requiring their qualification for the State-funded Community Care for the Elderly (CCE) Program (in-home and other community services can be made available with prior authorization from the funding agency). This Agreement provides one flexible spending allocation category for all funded contract services. The single spending authority simplifies EVSD's ability to earn revenue from reimbursement of all service units. The Council will be solely responsible for determining client eligibility and releasing eligible clients to EVSD for the provision of services.

The Council is not requiring EVSD to serve a specific number of seniors or to meet any other performance outcome measures. The Council is, however, requiring that EVSD ensure that persons served are not recipients of the same services through the CCE grant. While no performance measures are set by the Council, EVSD will utilize and apply the same performance measures outlined for the CCE program as follows: 97% of Adult Protective Services referrals are served within 72 hours; 90% of clients at “imminent risk” status are served; 66% of clients demonstrate improvement in Nutrition score; 63% of clients demonstrate improvement in Activities of Daily Living score; 62.3% of clients demonstrate improvement in Instrumental Activities of Daily Living score; 90% of Caregivers report ability to continue providing care; 73.9% of clients demonstrate improvement in Environment score; the average time in this program for probable Long Term Care Managed Care clients shall be less than 2.8 months.  These goals were met in previous grant years.

This Agreement is in the Areawide Council on Aging of Broward County, Inc.'s format and as such their representative will sign last following approval by the Board of County Commissioners.

The County Attorney has reviewed and approved this Agreement as to form.
