THE HUMAN SERVICES DEPARTMENT AND THE ELDERLY AND VETERANS SERVICES DIVISION RECOMMEND APPROVAL OF THE ABOVE ACTIONS.
These actions support the Commission Value: "Approaching human services collaboratively and compassionately, with special emphasis on the most vulnerable," and the Commission Goal: "Effectively advocate for and acquire municipal, state and federal financial support to equitably address health and human services needs of the entire community, through a truly coordinated system of care." Funding will allow the Elderly and Veterans Services Division (“EVSD”) to provide caregiver training and support services to Broward County residents who are family caregivers of individuals age 60 or older and family caregivers age 55 or older.
Retroactive approval is sought due to delays with obtaining necessary documentation from the funder to finalize the Agreement.
EVSD has served as the lead agency for aging services in the County since October 1, 1985. Through this Agreement, EVSD will provide caregiver support services under OAA Title IIIE. Preference will be given to those with the greatest economic and social need, particularly low-income older individuals and minorities, as well as those with limited English proficiency. Priority will also be given to family caregivers who provide in-home care for individuals with Alzheimer’s disease or related disorders.
Although no specific target outcomes are required, EVSD will document the following: percent of clients who remain at home or in the community instead of going into a nursing home; percent of Adult Protective Services referred clients in need of immediate services to prevent further harm served within 72 hours of reporting; average of monthly savings per client for home and community-based care versus nursing home care; percent of new service clients whose Activities of Daily Living (e.g., bathing, eating, dressing, etc.) assessment score is maintained or improved; percent of new service clients whose Instrumental Activities of Daily Living (e.g., shopping, managing money, cooking, etc.) assessment score is maintained or improved; percent of clients who receive community-based care and are at imminent risk of nursing home placement; percent of active clients eating two or more meals per day; percent of caregivers who self-report being very confident about their ability to continue to provide care at home after service intervention.
Funding for this Agreement is supported by OAA Title IIIE dollars distributed locally by the State of Florida Department of Elder Affairs through the Council and by the Council’s match dollars. As such, the Council is acting on behalf of the State of Florida. This Agreement was prepared in the Council's format and has been reviewed and approved as to form by the Office of the County Attorney; the Council will sign last following approval by the Commission. |
