A. MOTION TO APPROVE Agreement No. CB00006-15-2019 between the Areawide Council on Aging of Broward County, Inc. ("Council") and Broward County, retroactively from October 1, 2018 through September 30, 2019, providing an allocation of $757,288 for community-based services to frail elders; authorize the Mayor and Clerk to execute the Agreement; authorize the County Administrator to take all administrative and budgetary actions to implement the Agreement.

B. MOTION TO ADOPT budget Resolution No. 2019-126 within the Local Match Program Fund to appropriate revenue in the amount of $757,288 for community-based services.

Summary Explanation/Background

THE HUMAN SERVICES DEPARTMENT AND THE ELDERLY AND VETERANS SERVICES DIVISION RECOMMEND APPROVAL OF THE ABOVE MOTIONS.

This program supports the Commission's Value: "Approaching human services collaboratively and compassionately, with special emphasis on the most vulnerable" and the goals to "Effectively advocate for and acquire municipal, state and federal financial support to equitably address health and human services needs of the entire community, through a truly coordinated system of care" and to “Deliver evidence-based services to the public, and connect customers and their family members, to sustainable support, with special emphasis on financial support." By acquiring these local funds and parterning with the Council, EVSD is able to provide community-based services that allow frail elders to age in place in their own homes with their familial caregivers.

Retroactive approval is sought due to delays with obtaining necessary documentation from the funder to finalize the agreement.

This funding allows EVSD to provide case management, health support, and adult day care services to frail elders. Eligible functionally and/or cognitively impaired individuals, 60 years of age and over, receive appropriate elder care assistance to prevent, postpone, or reduce their placement in nursing homes and other institutions. Each County Fiscal Year, the Council makes local match funding available for case management and adult day care services for eligible seniors without requiring their qualification for the state-funded Community Care for the elderly (CCE) program. Services are made available to eligible recipients after the Council determines their eligibility and authorizes services.

The Council does not require EVSD to serve a specific number of seniors. The Council does, however, require EVSD to ensure there is no dual enrollment with either the CCE Program or the Statewide Medicaid Managed Care Long-Term Care Program. While no performance measures are set by the Council, EVSD utilizes the following CCE program performance measures: 66% of clients demonstrate improvement in Nutrition score; 63% of clients demonstrate improvement in Activities of Daily Living score; 62% of clients demonstrate improvement in Instrumental Activities of Daily Living score; 90% of Caregivers report ability to continue providing care; 74% of clients demonstrate improvement in Environment score; the average time in this program for probable Statewide Medicaid Managed Care Long-Term Care clients is less than 2.8 months. EVSD met these goals on a yearly basis.

This Agreement was prepared by the Council in its format and has been reviewed and approved as to form by the Office of the County Attorney; the Council will sign last following approval by the Board.

Fiscal Impact/Cost Summary:
This Agreement provides one flexible spending allocation category for all funded services and does not require any cash or in-kind match from Broward County.
