MOTION TO APPROVE Second Amendment to Agreement No. JC117-15-2018 between Areawide Council on Aging of Broward County, Inc. (Council) and Broward County (County), effective upon date of execution through June 30, 2018, for the purpose of updating contract language, modifying the contract term, and introducing a new provision regarding renewals; authorizing the Mayor and Clerk to execute same; authorizing the County Administrator to execute renewals and amendments which do not increase the total funding amount above $21,000,000, subject to approval of the same by the Office of the County Attorney; and authorizing the County Administrator to take all administrative and budgetary actions needed to implement the agreement.

Summary Explanation/Background
THE HUMAN SERVICES DEPARTMENT AND THE ELDERLY AND VETERANS SERVICES DIVISION RECOMMEND APPROVAL OF THE ABOVE ACTION.

This action supports the Commission's Value of "[A]pproaching human services collaboratively and compassionately with special emphasis on the most vulnerable," and the goal to "[E]ffectively advocate for and acquire municipal, state, and federal financial support to equitably address health and human services needs of the entire community, through a truly coordinated system of care." 

The Elderly and Veterans Services Division (EVSD) has been the Lead Agency for the CCE Program since October 1, 1985. EVSD administers the program and provides case management and support services to frail elders. EVSD also maintains vendor agreements with home health and other community agencies for the provision of in-home services, adult day care, medical supplies and emergency response systems to eligible elders requiring assistance to cope with daily living demands. These services ultimately prevent, postpone, or reduce older adults' inappropriate or unnecessary placements in nursing homes and other institutions.

Expected outcomes are as follows: 97% of Adult Protective Services referrals are served within 72 hours; 90% of clients at “imminent risk” status are served; 66% of clients demonstrate improvement in Nutrition score; 63% of clients demonstrate improvement in Activities of Daily Living (bathing, eating, dressing, etc.) score; 62.3% of clients demonstrate improvement in Instrumental Activities of Daily Living (laundry, finances, shopping, etc.) score; 90% of caregivers report ability to continue providing care; 73.9% of clients demonstrate improvement in Environment score. During the current grant period, these performance measures are being met.

This Second Amendment to the Agreement updates contract language, modifies the contract term, and introduces a new provision wherein the Council may renew the Agreement for a period not to exceed three years. These changes are intended to facilitate contract process and compliance with CCE guidelines and do not alter the original purpose of the contract or scope of services.

State CCE funds are distributed through the Council. As such, the Council is acting on behalf of the State of Florida. This Amendment is in the State of Florida’s format; therefore, the Council will sign last, following approval by the Board of County Commissioners. The County Attorney's Office has reviewed and approved this Amendment as to form.
