Requested Action
A. MOTION TO APPROVE Amendment No. 3 to Contract No. 34381-17 between the Broward Behavioral Health Coalition, Inc. (BBHC), and Broward County, retroactively from March 1, 2018 through June 30, 2018, amending the total contract to $5,393,004 to provide community-based behavioral health services, of which BBHC will pay $4,494,170, subject to the delivery of billing for services and $898,834 for "Uncompensated Units Reimbursement Funds;" BBHC, at its sole discretion and subject to the availability of funds, may pay Broward County, in whole or in part, or not at all; and requiring an additional $25,376 for a total of $391,454 in local matching funds; authorizing the Mayor and clerk to execute same; authorizing the County Administrator to take the necessary administrative and budgetary actions to implement this grant Agreement.

B. MOTION TO ADOPT budget Resolution No. 2018-247 within the BBHC Substance Abuse and Mental Health Grant Fund to appropriate revenue in the amount of $9,555 for the purpose of providing behavioral health services to eligible adult residents of Broward County.

Why Action is Necessary
A. Agreements with external agencies and budget Resolutions require approval by the Board of County Commissioners.

Summary Explanation/Background
THE HUMAN SERVICES DEPARTMENT, THE BROWARD ADDICTION RECOVERY DIVISION, THE ELDERLY AND VETERANS SERVICES DIVISION, AND THE CRISIS INTERVENTION AND SUPPORT DIVISION RECOMMEND APPROVAL OF THE ABOVE MOTION.

This item supports the Board's Value of "Approaching human services collaboratively and compassionately with special emphasis on the most vulnerable," and Goal to "Deliver evidence-based services to the public, and connect customers and their family members, to sustainable support, with special emphasis on financial supports." Funding from this agreement allows eligible County residents to receive behavioral health and support services.

Retroactive approval is being requested due to the fact that the contract amendment was received by Broward County on March 20, 2018 with an effective date of March 1, 2018. As a result, final review was delayed. 

This is the second year that services provided by the Broward Addiction Recovery Division (BARC), and Elderly and Veterans Services Division (EVSD) through the Florida Department of Children and Families (DCF) grants administered by the Broward Behavioral Health Coalition (BBHC) have been consolidated under one contract. The first amendment, executed August 7, 2017 during summer recess, added a third County agency to the Agreement with $126,000 in funding for the addition of the Juvenile Post-Arrest Diversion (JPAD) Program, currently being administered by the Crisis Intervention and Support Division (CISD). This amendment also provided $459,359 in additional funding for the Medication Assisted Treatment (MAT) Program administered by BARC. As authorized, the County Administrator executed the second amendment on December 13, 2017,  to revise the rate paid for MAT Program services.

This Amendment 3 reduces funding for EVSD and CISD services, while increases and reallocates funding for BARC services to address the opioid crisis. While the overall required amount of the Agreement is decreased by $15,821, the reallocation to non-exempt BARC services increases the required local match by $25,376. Match for this contract is based on the formula found in Rule 65E-14.005, Florida Administrative Code, which stipulates, when funding is revised match requirements may increase, decrease, or remain the same depending on which category of services are impacted by the funding change, as not all services require matching funds.

BARC has been serving adults with substance use disorders since 1973 and currently provides a full array of comprehensive substance abuse detoxification and treatment services in a community-based setting, and is designated by DCF to provide state and federally-funded behavioral health services to eligible Broward County residents. BARC provides medically-supervised residential detoxification, intensive residential treatment, intensive outpatient treatment, and outpatient individual and group counseling services. Outcomes for BARC remain unchanged from the original agreement and include: serving 1350 adults; 10% change in clients who are employed from admission to discharge; 15% change in the number of adults arrested in 30 days prior to admission versus 30 days prior to discharge; 51% of adults who successfully complete substance abuse treatment services; 94% of adults with substance abuse who live in a stable housing environment at the time of discharge; 100% of persons receiving state-contracted substance abuse service event records which have matching substance abuse initial (all purposes) admission records in the Substance Abuse and Mental Health Information Systems will be at least (SAMHIS). At least 50% of clients discharged from detoxification services will be connected to services in the BBHC Network at a lower level of care within seven days; recidivism rate shall be less than 50% within 90 days of discharge from detoxification. No more than 15% detoxification readmissions within 30 days; and BARC shall submit admission records that match substance abuse service event records in the BBHC Substance Abuse and Mental Health Information System database at a 100% rate.

EVSD has been providing behavioral health services to elders residing in Broward County since 1977. EVSD has been the sole provider in Broward County specializing in case management services for individuals 55 years of age and older meeting the State of Florida priority definition of having Severe Mental Illness (SMI). EVSD's Behavioral Health Section provides comprehensive assessment, targeted mental health case management, in-home/on-site counseling, screening, coordination of resources, and information and referral for elders with mental health disorders. Clients served include those with forensic involvement and EVSD works directly with the Broward County Mental Health Court to assist these individuals in assimilating into the community. Outcomes for EVSD remain unchanged and include: serve 155 adults with severe and persistent mental illness and 55 adults with forensic involvement. One hundred percent (100%) of persons receiving state-contracted mental health service event records which have mental health initial admission records in the Substance Abuse and Mental Health Information System (SAMHIS); Case Managers will attend at least 95% of the monthly concurrent reviews and follow through with 100% of the recommendations from prior month; Case Managers will attend at least 95% of the South Florida Monthly Discharge or treatment plan meetings at the State Hospital, as reported by State Hospital Staff.

CISD is a newly created division under the Human Services Department (HSD), consolidating the Office of Justice Services and the Nancy J. Cotterman Center. The Justice Services Section (JS) manages processes and coordinates equity and strength-based prevention and intervention services that redirect youth and adult offenders away from the criminal justice system. Juvenile Predisposition Services (JPS) is a program that is designed to immediately reduce the number of youth held in secure detention and to begin addressing individualized needs to prevent deeper involvement into the criminal juvenile justice system. The goal of JPS is to build a neighborhood-based continuum of care that keeps the community safe and support youth and their families by providing case coordination. JPS provides case coordination services for the newly created Juvenile Post-Arrest Diversion (JPAD) Program developed in partnership with the State Attorney's Office. Under Amendment 3, expected outcomes for children’s mental health services recipients are as follows: CISD will serve 20 children with serious emotional disturbances and ten children with emotional disturbances.

This Amendment 3 provides for a net increase in funding and local match in the amount of $9,555 and the changes are: EVSD from $506,510 to $319,510 for a decrease of $187,000;  CISD from $126,000 to $43,000 for a decrease of $83,000; BARC from $3,877,481 to $4,131,660 for an increase of $254,179; Total contract from $4,509,991 to $4,494,170 for a total decrease of $15,821. Local match requirement changes are: EVSD remains at $24,836; CISD remains at $0; BARC from $341,242 to $366,618 for an increase of $25,376; Net funding change (contract funding plus local match) - increase of $9,555

The Amendment is in BBHC's format, thus BBHC will sign last following approval by the Board of County Commissioners. This Agreement has been reviewed and approved as to form by The Office of the County Attorney.
