THE HUMAN SERVICES DEPARTMENT AND THE ELDERLY AND VETERANS SERVICES DIVISION RECOMMEND APPROVAL OF THE ABOVE ACTIONS.
These actions support the Commission's Value of "Approaching human services collaboratively and compassionately, with special emphasis on the most vulnerable," and the Goal to "Effectively advocate for and acquire municipal, state and federal financial support to equitably address health and human services needs of the entire community, through a truly coordinated system of care." Caregiver training and support services funded by this grant will enhance services to frail and low-income elderly who suffer from Alzheimer's disease and other related conditions.
The Elderly and Veterans Services Division (EVSD) has served as lead agency for aging services in the County since October 1, 1985. Through this Agreement, EVSD will provide caregiver support services under OAA Title IIIE. Services will be provided to family caregivers of individuals age 60 or older and to individuals age 55 or older who are relative caregivers. Preference will be given to those with the greatest economic and social need, particularly low-income older individuals and minorities, as well as those with limited English proficiency. Priority will be given to family caregivers who provide care for individuals with Alzheimer’s disease or related disorders.
Although no specific target outcomes were identified, EVSD must document the following: percent of most frail elders who remain at home or in the community instead of going into a nursing home; percent of Adult Protective Services referrals who are in need of immediate services to prevent further harm who are served within 72 hours; average monthly savings per consumer for home and community-based care versus nursing home care for comparable client groups; percent of new service recipients whose Activities of Daily Living (i.e., bathing, eating, dressing, etc.) assessment score are maintained or improved; percent of new service recipients whose Instrumental Activities of Daily Living (shopping, managing money, cooking, etc.) assessment score are maintained or improved; percent of customers who are at imminent risk of nursing home placement who are served with community based services; percentage of active clients eating two or more meals per day; after service intervention, the percentage of caregivers who self-report being very confident about their ability to continue to provide care.
The funding for this agreement is supported by OAA Title IIIE dollars distributed locally by the State of Florida Department of Elder Affairs through the Council. As such, the Council is acting on behalf of the State of Florida. This Agreement is in the State of Florida’s format. The Council will sign last, following approval by the Board of County Commissioners. The County Attorney’s Office has reviewed and approved this Agreement as to form. |
