THE HUMAN SERVICES DEPARTMENT AND THE ELDERLY AND VETERANS SERVICES DIVISION RECOMMEND APPROVAL OF THE ABOVE ACTIONS.
These actions support the Commission's Value of, "Approaching human services collaboratively and compassionately, with special emphasis on the most vulnerable," and the Goal to "Effectively advocate for and acquire municipal, state and federal financial support to equitably address health and human services needs of the entire community, through a truly coordinated system of care." Increased funding will provide additional services to Broward County residents who are 60 years of age or older, functionally impaired, and aging at home.
The Elderly and Veterans Services Division (EVSD) has been the Lead Agency for the CCE Program since October 1, 1985. EVSD administers the program and provides case management and support services to frail elders. EVSD also maintains vendor agreements with home health and other community agencies for the provision of in-home, adult day care, medical supply and emergency response system services to eligible elders requiring assistance to cope with daily living demands. These services ultimately prevent, postpone, or reduce older adults' inappropriate or unnecessary placements in nursing homes and other institutions.
Expected outcomes are as follows: 97% of Adult Protective Services referrals are served within 72 hours; 90% of clients at “imminent risk” status are served; 66% of clients demonstrate improvement in Nutrition score; 63% of clients demonstrate improvement in Activities of Daily Living score; 62.3% of clients demonstrate improvement in Instrumental Activities of Daily Living score; 90% of caregivers report ability to continue providing care; 73.9% of clients demonstrate improvement in Environment score. During the current grant period, these performance measures are being met.
This First Amendment to Agreement increases the state grant funding by $332,175, from $5,806,272 to $6,138,447 and the required match by $36,909, from $645,141 to $682,050. Consequently, it revises Section 4 of the Standard Contract and its Exhibit 2 regarding Contract Amount and Source of Funds. The Amendment also replaces Attachment IV with an updated Budget Summary. Lastly, it introduces Attachment VIII regarding the Annual Co-Payment Assessment & Collection Report. The new contract amount is expected to be recurrent and further augmented in upcoming years, thus elevating EVSD'S caseload and prompting the need to increase the grant-personnel cap by two full-time employees (FTE) within the CCE Grant Fund. For this reason, EVSD will be adding a Human Services Supervisor to provide supervisory and administrative case management and a Case Manager/Counselor to assess clients, authorize care plans and oversee provision of approved services. There are no vacant BPNs to utilize. These positions will expire at conclusion of the grant.
State CCE funds are distributed through the Council. As such, the Council is acting on behalf of the State of Florida. This Amendment is in the State of Florida’s format; therefore, the Council will sign last, following approval by the Board of County Commissioners. The County Attorney's Office has reviewed and approved this Amendment as to form.
